Certificates Of Insurance

All requests for certificates of insurance must be made in writing, and may be requested online by clicking here or via fax (215) 542-6990. The request must include:

The name of the condominium association
The name(s) of the borrower
The name of the lender
The lender’s mortgagee clause, including the address the lender wants shown on the certificate
A return email, fax number or mailing address where the certificate should be forwarded

Commercial Claims must be submitted in writing to nstewart@smithinsurance.com or to fax number (215) 542-6990. If you live in a Community Association, the claim must be reported to us by your Property Manager or an active Board Member.
